Valid Passport<\/strong>&nbsp;not less than 6 months and has at least TWO completely empty pages<\/p>\n\n\n\n<p><strong>2. Visa Application Form and one colour photograph taken within 3 months&nbsp;(size 3.5\u00d74.5cm)<br><\/strong>\u2022&nbsp;Completely filled out and signed as the same as the applicant&#8217;s passport signature<\/p>\n\n\n\n<p><strong>3. Personal history form<\/strong>&nbsp;completely filled in and signed by the applicant.&nbsp;<\/p>\n\n\n\n<p><strong>4. One original bank statement&nbsp;in English<\/strong>&nbsp;<br>\u2022&nbsp;The bank statement must contain the amount at least 20,000 Baht per person<br>\u2022 In case of applicant under the age of 20 years who do not have their own bank account is required to submit a bank statement of either father or mother with the amount at least 40,000 Baht per family, and one original family registration certificate in Japan (KOSEKITOHON) which is issued within 3 months<br>\u2022 Copy of bank book is not acceptable<\/p>\n\n\n\n<p><strong>5.&nbsp;<\/strong><strong>Evidence of accommodations for the entire duration of stay in Thailand<\/strong>, please provide one of following documents&nbsp;<br>5.1 Hotel booking confirmation&nbsp;<\/p>\n\n\n\n<p>5.2 Copy of applicant&#8217;s own evidence for&nbsp;condominium \/ apartment \/ house<\/p>\n\n\n\n<p>5.3 In case of renting&nbsp;condominium \/ apartment \/ house : Copy of the lease agreement and landlord&#8217;s ID card is required<\/p>\n\n\n\n<p>5.4 In case of&nbsp;staying with friend or family, the following documents from A-D is required<br>&nbsp; &nbsp; &nbsp; &nbsp;A. Invitation Letter from friend or family (including name of the writer, relationship with the applicant, address for staying, length of stay, contact, and signature of the writer)<br>&nbsp; &nbsp; &nbsp; &nbsp;B. In case of owning or renting condominium \/ apartment \/ house, a copy of the applicant\u2019s own evidence or the lease agreement<br>&nbsp; &nbsp; &nbsp; &nbsp;C. One copy of Identification Card (ID Card) of friend or family<br>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; \u2022 In case of Thai national, must submit a copy of ID card and House Registration which include address and name<br>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; \u2022 In case of foreigner, must submit a copy of passport (photo page and valid visa pages) and, in case of foreigner legally working in Thailand, must also submit a copy of valid work permit<br>&nbsp; &nbsp; &nbsp; &nbsp;D. Official documents proving family relationship<br>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; \u2022 For Japanese national, must submit one original family registration certificate of Japan (KOSEKITOHON) issued within 3 months<br>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; \u2022 For foreigners, must submit one copy of Registration of Marriage with English translation or Birth Certificate with English translation<\/p>\n\n\n\n<p><strong>6. Occupation\u2019s documents&nbsp;<\/strong><br>6.1&nbsp;<strong>For company employee<\/strong>, one original certificate of employment, certificate of leave and one original copy of company\u2019s registration (TOKIBOTOHON) are required<\/p>\n\n\n\n<p>6.2&nbsp;<strong>For self-employment<\/strong>, one original company\u2019s registration (TOKIBOTOHON) is required<\/p>\n\n\n\n<p>6.3&nbsp;<strong>For student<\/strong>, one original certificate of enrollment issued by the academic institute in Japan is required<\/p>\n\n\n\n<p>6.4&nbsp;<strong>For pensioner<\/strong>, one original certificate of pensioner\u2019s fund stating an amount of pension received is required<\/p>\n\n\n\n<p>6.5&nbsp;<strong>For housewife \/ househusband<br><\/strong>\u2022 Japanese applicant is required to submit one original family registration certificate of Japan (KOSEKITOHON) issued within 3 months and one copy of photo-page passport<br>\u2022 Non Japanese applicant with Japanese spouse is required to submit one original of the spouse\u2019s family registration certification of Japan (KOSEKITOHON) issued within 3 months and one copy of photo-page passport<br>\u2022 Non Japanese applicant with non Japanese spouse is required to submit one copy of registration of marriage with English translation or birth certificate with English translation<\/p>\n\n\n\n<p>6.6&nbsp;<strong>For applicants under 20 years old<\/strong>, one original guarantee letter and a copy of guarantor&#8217;s passport or driving license duly signed by the guarantor are required<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table><tbody><tr><td><strong>&nbsp; &nbsp;Guarantee Letter (Form as above link)<br><\/strong>&nbsp; &nbsp;\u25cf A guarantor must be twenty (20) years of age or older, must be able to confirm the applicant&#8217;s personal attributes. A guarantor must be available to be contacted by the Consulate-General and must not be a person who will accompany the applicant to Thailand<br>&nbsp; &nbsp;\u25cf A guarantor who are not Japanese national must provide a copy of passport and residence card (legally residing in Japan)<br>&nbsp; &nbsp;\u25cf A guarantor must sign as the same as the guarantor&#8217;s identification document signature<br>&nbsp; &nbsp;\u25cf Japanese guarantor who prefers to use driver license, please sign as the same as signature in the guarantor letter on the copy of driver license&nbsp;<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p>6.7&nbsp;<strong>For unemployment<\/strong>,<br>&nbsp; 1) One original certificate of personal income tax \/ original certificate of personal income tax exemption or similar document.<br>&nbsp; 2) One original guarantee letter must be presented together with a copy of guarantor&#8217;s passport&nbsp;or driving license duly signed by the guarantor.&nbsp;<br>*A guarantor must be twenty (20) years of age or older and a permanent resident legally residing in Japan. A guarantor must not accompany the applicant to Thailand, while must be able to confirm applicant&#8217;s personal attributes and information such as name, age, occupation, address, physical information, etc. and available to be contacted by the Embassy.&nbsp;<\/p>\n\n\n\n<p><strong>7. English itinerary and purpose of visit<\/strong><\/p>\n\n\n\n<p><strong>8.
